WHAT INSURANCE DO YOU TAKE?

We are an in-network provider for BCBS of IL PPO plans as well as Cigna PPO plans. We also accept Out-of-Network clients and can provide you with a bill for reimbursement from your insurance.

Clients who do not have or wish to use insurance are also accepted.

WHAT ARE YOUR RATES FOR SESSIONS?

  • A 55-60 minute Initial Individual Intake Session will be billed at the rate of $200

    Note: If you are using insurance, you will still only pay the contracted deductible amount or co-pay amount if you’ve hit your deductible

  • Subsequent 50-55 minute Individual Therapy sessions are billed at the rate of $125

  • Haven Therapy & Wellness is considered out-of-network with any insurance providers that aren’t BCBS PPO or Cigna PPO - we are not in network with HMO plans. Out-of-network means that we do not have a contract with your health insurance plan provider. However, it is still possible to use these benefits.

    At the time of service, you pay the fee directly and we will provide you with a monthly “Superbill” to submit to your insurance provider for reimbursement. The reimbursement is paid directly to you. For more information and greater detail regarding your benefits, please call your insurance provider about coverage for our services.

WHAT METHODS OF PAYMENT DO YOU ACCEPT?

Clients can pay directly using the client portal via credit, debit, FSA, or HSA card. Payments are automatically charged on day of service or when insurance claim has been processed. You can be provided with an invoice upon request.

I’ve never done therapy before - what can I expect?

During the intake session, your therapist will go over the therapy process and you will share what you’d like to work on in therapy. The first few sessions are a sharing of social history and relevant information for the topics you’d like to cover in therapy, goal setting, and building rapport with your therapist. During this time, your therapist will also be providing you with education on what treatment will look like. Sessions occur weekly for 55 minutes (that is the time that insurance considers a “therapy hour”). Some clients require more frequent sessions (2x a week), but any more than that is not available in outpatient settings and would require a referral to a more intensive treatment center. Clients typically experience improvement in symptoms within 8-12 weeks, but it is specific to each client how long treatment lasts.

DO YOU OFFER IN-PERSON SESSIONS?

Yes, I see clients in-person at 1623 N. Western Avenue in Chicago on Mondays and Tuesdays. This office is located near the intersection of Western Avenue and North Avenue in the Wicker Park/Bucktown area. There is free street parking available and it is close to both the Western Blue Line stop and the 49 Western buss (Western & North stop). Clients seen in person are provided with a key code to enter the building securely. The building is ADA accessible.

For clients that are seen via telehealth, we use a HIPAA-compliant telehealth platform. Telehealth clients are seen on Wednesdays and Thursdays.

WHAT IS YOUR CANCELLATION POLICY?

Clients can cancel or reschedule at any time as long as they provide 48 hours notice. Cancellations may be subjected to a charge of the full session fee if not received with at least 48 hours notice. These fees would be out-of-pocket and cannot be charged to a FSA or HSA. Fees may be waived due to unexpected emergencies such as serious illness.

DO YOU SEE COUPLES OR KIDS?

No - I only see individuals over the age of 18.
